Business
Sysmex Corporation, founded in 1968 and headquartered in Kobe, is a medical device and reagent manufacturer specializing in in-vitro diagnostics (IVD). Centered on Hematology (Blood Cell Counting), the company provides an integrated offering of instruments, reagents, software, and services across a broad range of clinical testing fields, including blood coagulation testing, urinalysis, and genetic testing. The group, comprising 80 consolidated subsidiaries, operates sales and service networks in over 190 countries worldwide, with major customers including hospitals, clinical laboratories, and other medical institutions. For FY2026 (ending March 2026), net sales are ¥500,006 million, with overseas sales accounting for 88.3% of the total. The company is also expanding into therapeutic areas such as surgical support robots (hinotori™) and regenerative cell medicine.
Business Model
The company adopts a "razor-and-blade" recurring revenue business model, expanding its installed base by selling and leasing clinical testing instruments to healthcare institutions, then capturing ongoing demand for consumable testing reagents and maintenance services that follow. Instrument sales generate initial revenue, while reagents and services provide stable, recurring revenue. Expansion of the installed base on a global scale structurally strengthens the long-term revenue base. Long-term alliance agreements with Siemens Healthineers and Roche also supplement revenue through OEM supply and global partnerships.
Company Strengths
Since its founding, the company has accumulated technology by specializing in the hematology (blood cell counting) field for over 50 years, and has built a sales and service network spanning more than 190 countries worldwide. In FY2026 (ending March 2026), the overseas sales ratio reached 88.3%, and the company maintains a stable global earnings base through its four regional headquarters system covering the Americas, EMEA, China, and AP. A customer base and brand strength that competitors cannot easily replicate in a short period form the core of its strength.
Recurring revenue is generated from reagent consumables and maintenance services that continue to arise after equipment installation, enhancing the stability of business performance. In FY2026 (ending March 2026) as well, the significant decline in revenue in China (down 24.2% year on year) was offset by increases in the Americas (up 6.2% year on year), EMEA (up 12.6% year on year), and AP (up 6.4% year on year), and total overseas sales of ¥441,403 million remained almost flat compared to the previous period.
The company has obtained an issuer rating of AA- from Rating and Investment Information, Inc. (R&I), enabling low-cost fundraising. At the end of FY2026 (ending March 2026), the ratio of equity attributable to owners of the parent was 71.4% (up 1.7 percentage points year on year), and total assets stood at ¥707,532 million, indicating high financial soundness. Operating cash flow of ¥73,848 million serves as the main funding source for capital expenditure and research and development, covered by the company's own funds.
ENVALITH's Perspective
Performance Trend
Revenue grew for four consecutive fiscal years, from ¥363,780 million in FY2022 (ended March 2022) to ¥508,643 million in FY2025 (ended March 2025), but FY2026 (ending March 2026) marked the first revenue decline, coming in at ¥500,006 million (down 1.7% year on year). Operating profit fell sharply to ¥51,831 million (down 40.8% year on year), and profit attributable to owners of parent dropped to ¥35,457 million (down 33.9% year on year). The main causes were a 24.2% decline in revenue in China due to that region's medical cost containment policies, the recognition of ¥11,557 million in impairment losses associated with changes in the business environment and strategy, and an increase in SG&A expenses tied to digital infrastructure investment. As for external factors, the ongoing yen depreciation (foreign currency translation adjustments of ¥30,410 million from overseas operations) boosted assets and capital, while policy risk in China was the primary factor weighing on performance. Operating cash flow remained at a high level of ¥73,848 million, indicating that cash-generating capacity has been maintained.
Growth Strategy
Aiming to strengthen Diagnostics competitiveness, improve profitability, and enhance capital efficiency through four themes under the new medium-term management plan
The company is expanding its product portfolio through the domestic and overseas rollout of the CN Series Automated Blood Coagulation Analyzer in the blood coagulation field, the global rollout of new Hematology products, and entry into the biochemistry testing field through the acquisition of JEOL's medical equipment business. The contribution of the CN Series is explicitly reflected in the forecast for FY2027 (ending March 2027).
By promoting medical DX and data utilization through ICT solutions such as Caresphere™, the company aims to enhance customer value and differentiate itself from competitors. Investment in building the digital infrastructure is a factor behind the increase in SG&A expenses in FY2026 (ending March 2026), and mid-to-long-term earnings contribution is expected.
As a key theme of the new medium-term management plan (FY2027 (ending March 2027) to FY2029 (ending March 2029)), the company will review each part of the value chain—manufacturing, procurement, and sales—with the aim of improving the cost of sales ratio and profit margin. In FY2026 (ending March 2026), the cost of sales ratio rose, putting the effectiveness of the reform to the test.
The company is promoting stronger cost competitiveness in the Asia Pacific region through the full-scale operation of the new production site in India (completed August 2024), and expanding the urinalysis field in Latin America. The ¥50,000 million borrowing in April 2026 explicitly cites Latin American business expansion as its intended use, indicating that capital deployment is now in full swing.
This is the fourth key theme of the new medium-term management plan. The company is strengthening shareholder returns through share buybacks (¥3,214 million in FY2026, ending March 2026) and dividend increases (annual dividend of ¥38, payout ratio of 67.3%), while also promoting improved capital efficiency to enhance ROE. The rise in the dividend payout ratio is premised on a recovery in profit levels.
